J Kids Enrollment Forms

All J Kids applicants need to complete the following forms:

• Emergency Form – Must be completed, signed and dated by parent.
• MSDE Medication Authorization – Must be completed, signed and dated by parent.
• MSDE Health Inventory – Must be completed, signed and dated by both parents and health practitioner.
• MDH Immunization Certificate – Must be completed, signed and dated by health practitioner.
• Assumption of Risk - Must be completed, signed and dated by parent.
• Policy Waiver - Must be completed, signed and dated by parent.
• Confidential Student Profile – Must be completed and submitted by parent.
• Virtual Learning Schedule – Must be completed and submitted by parent.
• Authorized Grown-Ups – Must be completed and submitted by parent.
